In today’s fast-paced medical environment, fragmented paper records and scattered digital files are no longer acceptable. Pensieve introduces a revolutionary Patient Records system that places every single clinical event - from the first chief complaint to the final discharge summary - in perfect chronological order. Every patient ever registered in patient will have only one health record. Same patient can’t have more than one health record. Health records are linked with AADHAR or the mobile number. One mobile number can be used for multiple health records. All the health record related to clinic visits is stored chronologically as per visits. Apart from this, there is complete ledger of all the scheduled and random actions taken by healthcare staff for the patient. It gives a complete picture of the patient’s health. Every piece of data is captured through intelligent, reusable templates: vital signs, nursing assessments, IP progress notes, surgical notes, physiotherapy records, discharge summaries, and hundreds of custom forms built by doctors themselves using the in-app no-code Template Builder. Once saved, the entry instantly appears on the patient’s timeline with exact timestamp, author name, department, and digital signature. Demographics, allergies, past history, family history, and immunization records live in a single master Health Record document, while all dynamic data (diagnoses, orders, investigations, prescriptions, procedures). Patient data is fully searchable, filterable and can be used for context in built-in AI chat tool of Pensieve. Patients are in control of their data. They can authenticate sites and teams to view and edit their data. Data of cross-site visits is also available. Because everything is real-time and cloud-native, the same timeline is visible simultaneously to the treating doctor, nursing station, pharmacy, billing desk, and the patient themselves through the Patient App (configurable) - ending the era of “phone calls to check status”.
